How to Apply for Disability Services
  1. The student is responsible to initiate a request for accommodations by applying for Student Accessibility (SA). An application can be requested either in person, by telephone, or Complete and Submit the Application for Student Accessibility Form.
    In order to receive accommodations, the student is required to provide current documentation. (View documentation guidelines) of the disability as well as the need for reasonable accommodation. In requesting accommodations that require time to prepare, such as electronic textbooks or hiring sign language interpreters, requests need to be made early enough to allow ample time to fulfill the request, whenever possible, 60 days before the beginning of the semester.

  2. The student meets with the SA coordinator to do the following:
    1. Provide initial information about him/herself.
    2. Review documentation.
    3. Discuss needed accommodations.
    4. Discuss policies and procedures.
    5. Complete appropriate accommodation forms for faculty.

  3. The SA coordinator will provide letters to instructors verifying a disability and stating the reasonable and appropriate accommodations recommended by the Student Accessibility office. The student is responsible for delivering the letter to the instructor as soon as possible at the beginning of the semester.
